UWCI Accreditation Annual Financial Submission Form

Thank you for submitting your annual financial information as an accredited partner. Per the accreditation agreement, all partner’s must submit the following items to UWCI:

  1. Most Current Form 990
  2. Most Current End of Year Financials (May be the organization’s choice of audited, reviewed, compilations, or internal end of year financials).

 

If your organizations has any  other changes that require communication to UWCI, please complete the Community Partner Contact Information form. Changes that can be submitted include:

  • Changes in executive and/or board leadership
  • Organization Name Change
  • Address Change

 

Financial Information must be submitted by July 31st. Please email accreditation@uwci.org with any questions.
